Before clinicals can be approved, we need your attention and action regarding the following:

The Illinois School Code requires all school employees and other agents who may have direct and/or daily contact with students in grades PK – 12 to have a fingerprint background check. To come into compliance, we need all University employees who visit the Laboratory Schools and may have direct and/or daily contact with students to complete a fingerprint background check.

 We are now accepting Professor Request Forms for Clinical Experiences. These must be approved before your students may begin their clinical experiences.

• Instructor Request Form for Clinical Experiences
  • Please denote how many hours of each type of clinical experiences  (example: 4 hours of observations, 2 hours of whole class instruction (6 hours total))
