Fingerprint Background Check
The Illinois School Code requires all school employees and other agents who may have direct and/or daily contact with students in grades PK – 12 to have a fingerprint background check. To come into compliance, we need all University employees who visit the Laboratory Schools and may have direct and/or daily contact with students to complete a fingerprint background check.
Professor Request Forms for Clinical Experiences
We are now accepting Professor Request Forms for Clinical Experiences. These must be approved before your students may begin their clinical experiences.

Please denote how many hours of each type of clinical experiences (example: 4 hours of observations, 2 hours of whole class instruction (6 hours total))
